1	11/10/2011	At about 4.45 PM he expressed uneasiness. In meanwhile arranging conveyance, he left the mine on a Motor Cycle to a Private Hospital without informing to the Foreman. After attending the Doctor again he return to the Workshop about 6.30 PM. At about 7.00 PM he started vomiting and left the mine by 7.00PM. Again he went to a private hospital along with his wife and he was declared dead at about 11.30PM.
