1	8/4/2009	After completion of his duties at 1R district, he was traveling along travelling road way and found in unconscious state at 13R/112LS by 3rd shift workmen. He was immediately brought to surface and sent to Main Hjospital where he was declared brought dead.
2	5/13/2009	After completion of his duties, he was traveling along 10R/79L in Top seam. He felt discomfort and collapsed at 10D/79L at about 10.45 p.m. Immediately brought to surface by co workmen, where he was declared dead at about 12.30 a.m.
