Sl.No Accident Date Description
1 11/26/2012 While Motor Grader No.6 was going on haul road in upward direction, suddenly engine was stopped. Motor Grader operator got down from equipment and was trying to put stopper behind the tyres. At a moment it ran back and rode over side berm. No one was injured in this incident.
2 11/22/2012 At about 3.15 p.m. a piece of stone ejected during blasting and fallen over the roof of Base workshop (Auto electrical section)and hit to iron beam and broken into smaller pieces. At that time no one was present at Auto electrical section. Distance from the blasting site to spot of fallen piece of stone is 710 mtrs i.e. beyond danger zone.
3 10/12/2012 While removing 120 Sq.mm.idle power cable with help of Escort No.3 crane(ACE) total cable was lifted out and while trying to make a turn to load on lorry Escort crane was toppled.Operator was safely escaped without any injury.
4 4/25/2012 On 25.4.2012 at about 9.00 am,empty Bowser No.1 (AP15T 6456)was coming through main haul road to surface (30-40mts from surface) the front left side tyre came out due to axil hub bearing damaged at phase-II of ocp-I.Nobody injured in this incident.
5 3/31/2012 While reversing of No.KH-4(100T)dumper for unloading OB material at dump yard, suddenly rear left inner tyre got burst.Due to which one piece of sand stone ejected and hit to No.KH-8(100T)dumper, causing front wind shield glass damaged.
6 3/13/2012 While volvo no.64,operated by sri.T.Durgaiah ,was going to Adriyala dump yard from Off-loading quarry,hit to back side of Volvo no.66.which was stopped aside on haul road due to rise in engine temperature due to which left side of the cabin of Vol no.64 got pressed in side and operator escaped without injuries.
