1 12/7/2010 empty 35T dumper No;H-26 coming from the dump yard hit another emty 35T dumper No;H-25 which was stopped near a crossing on the same haul road from back side due to not maintaining the safe distance between the HEMM.
2 12/6/2010 while 100T dumper No;BH-157 is being topped up with HSD,the operator has moved the dumper suddenly without any signal from the deisel filling crew endangering the safety of the person who was filling the deisel in to the dumper.
3 10/26/2010 While a fitter helper was repairing B-6 rope shovels bucket teeth point adaptor, the shovel operator suddenly raised the bucket without giving any warning alarm,the fitter helper narrowly escaped from any injury
4 10/25/2010 while toeing the breakdown crane(5T)from the quarry to the workshop with the help of 75T Coles Crane, the telescopic boom came in contact with the 33KV HT Over Head line which was coincidently under maintenance at that time.
5 10/21/2010 While reversing the 100t dumper(BH-4)in dumpyard to unload the OB,hit working dozer(No:15)causing damage to the dozer.Dozer operator ascaped from injury.
6 10/20/2010 Breakage of fluid coupling due to misalignment caused by the damage of multidisc on Q-3 quarry belt drive unit bellhouse in OC-1,CHP.Broken parts were thrown away along with gaurd plate provided.
7 8/11/2010 Due to failure of active dump yard slope up to 15m from the edge,a loaded 100T dumper(BH-161) which was ready to unload the OB, slided down along the plane of failure up to 3mtr depth.Presence of the silt in the bottom of the toe is the reason for the slope failure.
8 7/28/2010 while going in reverse direction to dump the OB in B-5 dumpyard BH-169 (100T)dumper bogged down and leaned towards the edge of the dump yard due to loose and uncompacted material in the floor. The dumper was pulled out with the dozer
9 7/25/2010 while dozing over the 3seam in 5th cut D-6 Dozer bogged about 1.5m down due to pot holing over 16L U/G gallery near 28dip junction.Dozer was removed safely with the help of another Dozer.
10 6/9/2010 while dressing the coal bench highwall(8mtrs height) by positioning the shovel closer to the high wall and on a low height platform,a big coal lump fell from the high wall on the TH-2 shovel stick,there by damaging the bucket cylinder.
11 5/28/2010 35T dumper No:50 with coal load overrided the stopper girder and slided in to the surface feeder breaker hopper in the process of unloading the coal in to the crusher.
12 5/6/2010 loaded 85T dumper No:101 coming from B-3 shovel to dumpyard tried to overtake a 35T dumper which was moving slowly near dragline junction on the same haul road,and over rided the divider and leaned towards empty road.
13 3/4/2010 While load Volvo No.50 was stopped aside in load haul road in off-loading area due to some report meanwhile another loaded Volvo No.32 coming from the quarry hit the Volvo No.50 from backside damage occure to Volvo No.32 no body injured in this incident.
14 1/9/2010 While dumper No.151 of 100T was returning from the dumpyard after unloading the OB material on empty haulroad hit the rear side of the Grader No.7 which is working on the haulroad causing damage to the grill and radiator of the Grader.
15 1/6/2010 While dumper No.75 of 85T is coming from the dumpyard after unloading the OB material on empty haulroad smoke was appeared on the exhaust tube though exhaust blanket is provided. Immediately the smoke was stopped using fire extinguisher and water tanker.
