1	5/14/2012	While the offloading Operator belongs to M/s. Sushee Infra Pvt. Ltd carrying the loaded Benz No: 12, to Coal dump yard at about 6.45 AM, he got drowsiness and drove the vehicle over the divider, due to the imbalance of the vehicle it got tilted slowly to the left side of the Operator.
2	2/13/2012	On 13.02.2012, in Ist shift at 2.45 PM while Sri K. Rajesh, Operator who was operating the Scania No: 24, at Internal ramp preparation, he made sharp turn which resulted in tilting of Scania to Right side of the operator. No injuries were received to the operator.
