1	5/14/2012	on 12.05.2012 in III shift at about 5.00 AM 30D/6Seam haular operator hauledup tubs on to haular drum.
2	5/3/2012	On 03.05.2012 in II shift at about 9.30 PM 17xR/AL Jn. trammer allowed 5 loaded tubs ranaway uncontrolly towards 16xD/AL Jn. that time 16xD haulage rope was raising loads from 2L and there tubs damaged bridge rails at 16xD and pulled the rope towards luckly rope was not snapped and person present in 16xD haulage roadway were escaped.
3	2/17/2012	On 17.02.2012 in I shift in 6L/3seam endless 8 no of loaded tubs ranaway uncontrolly from 39D to 35D from which 35D/6L junction trammer and two linemen were norrolly escaped.
4	2/7/2012	On 07.02.2012 in 1st shift at about 9.30AM, side fall of coal layer mesuring 8mX2mX1.5m size taken place at 17LN/18D/5eam from which 4 no.of coal fillers were narroully escaped.
5	1/21/2012	On 21.01.2012 in middle shift at about 3.00 PM side fall of coal layer of 4mX2mX1m size taken place at 17LS/19D/6Seam from which timbermen narroully escaped.
