1	12/20/2012	While he (U.Srinivas, Driver)was going to dump yard after taking OB load in Scania No.18 from 1 seam exposing OB bench (RL-825 in OC-II East side), the tipper run over the devider and toppled to left side of driver Sri U.Srinivas due to his negligence operation in sleeping condition
2	12/10/2012	The tyre mounted drill got toppled in the drilling area due to non-lowering of drill mast while marching from one location to another location and rear tyres uneven floor.
3	4/27/2012	Atlas Capco drill machine of offloading agency (VPR) got fire due to electrical wiring short circuit.
4	2/6/2012	While unloading muck from Scania No.18 for divider preperation at OC-II ramp it tilted to right side due to improper placement by the operator caused miner injury to right palm.
5	2/3/2012	Due to shot circuit of leads (between battery and self starter) smoke came from beneath seat of the dozer No.12657 (D.155) at RL 790. Immediately smoke was quenched
