1	4/16/2012	While hauling of loaded tubs from 21L/77D, 77R Hauler return pulley came out of its bracket shaft. This resulted into run a way of loaded tubs, entangling of Haulage rope to hauler brake post which caused snapping of rope, subsequently derailing of loaded tubs at 77D/21L. In this incident, while escaping from driver?s seat, the Hauler operator Sri Sirangi Narsiah received minor injury.
