1 9/8/2012 On 08.09.2012 in 1st shift beginning at about 8.10 AM, HD/39L Junction was found collapsed along with roof bolts with 5 girders. The man riding train guard observed the fall in trail run and stopped and informed to the superior.The HDip was developed with 3m working thickness between the years 2001 to 2002.And the man riding system was introduced in the year 2005.
2 6/11/2012 Rescue team went to Shaft bottom with breathing apparatus to find out environmental condition at the shaft bottom. As the shaft was closed from 19.05.2012.
3 5/24/2012 On 24.05.2012 in 3rd shift at about 4.30 AM the chain pulley block 5Te capacity hook attachment pin was broken while lifting an electrical motor of conveyor drive head at BDip/42L bunkers(motor weight is 1200 Kgs)
4 5/19/2012 A breakage of winding engine second motion pinion shaft, occurred on 19.05.2012 in 1st shift at about 10.00 AM and stopped the winding operations until replace the same
5 4/18/2012 On 18.04.2012 in 3rd shift at about 3.45 AM (early hours of 19.04.2012) the incident was occurred due to breakage of winding engine first motion pinion shaft, the 9 workman are held up in the shaft bottom, these persons were brought to surface by using of scaffold platform safely and the same was rectified with another pinion and winding engine was re-commissioned at 11.30 PM on 19.04.2012.
6 1/12/2012 in Ist shift main belt joint separated and cutted no incident recorded
