Sri K.RajuBabu Benz truck driver, of truck No B-51 (Reg No: AP 01W 7586) belonging to M/s Sushee infra Ltd Offloading Contractor,while proceeding to dumpyard No-1 with load, on excess watered haul road, the truck tyres got slipped and dragged back though brakes were applied and right side rear tyres got on to the divider resulting truck toppled to the left side.
Due to the spillage of hydraulic / Fuel / engine Oil over the heated engine, truck caught fire and burnt partially. |

, Benz Operater,While he was un-loading OB material from his Benz Truck No.59, he received heavy Jerk, due to sudden fall of Truck bucket after lifting the loaded bucket. He became panic/shock.
Due to shearing of Lock bolt in between Lock plate and dummy plate of bucket hoist cylinder, causing sudden discharge of hydraulic oil, resulting sudden fall of bucket with OB matetial. Due to which impact developed on the chassis causing Jerk/shock to the operator. |

Cause:-Sri. B. Srinivas shovel operator, while he was loading OB material in a truck with shovel(volvo 460-2),suddenly he observed some abnormal sound in the shovel body near main control valve area and in turn noticed smoke from main control valve in the body
Reason:-One of the hydraulic hose of control valve or diesel hose might have punctured/damaged resulting spraying of oil/diesel on turbo charger causing fire. The fire was quenched immediately with fire extinguishers, sand and water. |

While Sri M.Lingaiah, Welder doing gas cutting work at surveyed off IR drill machine cabin, the cabin got slided down , during that process his left back got scratch and got injury to his back. |
